Forms & Fees 

Below you will find the required forms for your Magdalen student athlete. All sports forms and fees are due to the Athletic Director, or the School Office before your child may practice. The sports physical is required annually dated after May 1 (ex: May 1, 2025 for the 2025-2026 Academic Year).

The ability to pay should not be a barrier to participating in athletics at Magdalen, so if you or someone you know needs consideration please contact the school administration or the athletic director.
